How to Request a Service
1. Complete the Service Form
Download the service request form and fill it in using block letters. Include your contact details, return address, equipment model and serial number, and the reason for service. Please tick the relevant service options and describe any faults or issues where applicable.
2. Return the Completed Form
Send the completed form to Fusion Innovations along with your equipment, following any delivery or courier arrangements confirmed with our team.
3. We’ll Assess Your Equipment
Once your equipment and service request have been received, our team can review the details and determine the appropriate next steps.
What Services Can You Request?
Calibration
Calibration services for compatible equipment, including Radiodetection eCert calibration for supported CAT and RD locators.
Repairs
Equipment repairs where you are experiencing a fault or your equipment is not operating as expected. Please describe the issue clearly on the form.
Servicing
Service requests for supported fusion and underground locating equipment.

Please check that you have completed the required sections of the form and included the correct equipment details. Providing an accurate model and serial number, along with a clear description of any faults, will help our team process your request.
